Stonerise is a network of transitional care services helping patients achieve the best possible outcomes so they can get back to health and back to life.
Throughout West Virginia and southeast Ohio, our nearly 3,000 team members provide personal, loving, clinician-led care to more than 2,000 patients. This includes transitional and skilled nursing care centers, therapy services, home health services, and hospice care. We provide care through these services across The Stonerise Network.
As we innovate and create positive change with our model of transitional care, we rely on the longstanding spirit of Appalachia, which means we strive to care for our patients, one another, our neighbors and our communities like we do our own families. Galatians 5:13 says, “Through love serve one another.” This is our purpose, and it guides every interaction we have with our patients, their families, our team members, our partners and the communities we serve. We believe it is our calling to serve as caregivers, and while some days are challenging, it is an honor to serve.
We believe that every team member is just as important as the next. We want every team member to succeed in a way that fulfills them personally.
We also believe that healthcare shouldn’t be a frustrating or fragmented experience. Our founders set out to do things differently when they started Stonerise. They wanted to create a bold model for transitional care—one that meets each patient where they are in their healthcare journey.
We know patients want more convenience, more empathy, more privacy and more support. When there are transitions in care, they want someone who knows them—and the healthcare world—to be alongside them. And they want people to do more than dispense care. They want people to play a loving role in their journey.
We’re dedicated to doing what is right and best for our patients. We regularly do more than what’s expected to improve someone’s health physically and emotionally. That’s more than healthcare. That’s love.
