iCare Health Network
Through its managed care centers, iCare Health Network (IHN) offers a range of services including medical, short-term rehabilitation, long-term care, behavioral health, substance use management, and memory care & support services. iCare provides management, operating and consulting services to skilled nursing facilities and other healthcare providers.
iCare operates 11 sites in Connecticut and 1 site in Massachusetts, including four Touchpoints Rehab centers, MissionCare at Holyoke, seven Greater Hartford Memory Care Centers, and SecureCare Options/60 West. IHN also offers LTC Integrity audit tools, separate outpatient therapy, rehabilitation and home care units and specializes in healthcare real estate transactions and skilled nursing turnarounds.
Previously known as iCare Management or just iCare, the management philosophy includes innovative clinical programming, strong financial and human resources management and using hands-on experience to cultivate sound operations. iCare provides a valuable community service role in serving underserved individuals who suffer from complex conditions and often have difficulty in locating other appropriate sites of care.
