Catholic Home Care (CHC), founded by The Congregation of the Infant Jesus in 1905, is a non-profit, non-denominational, home care agency certified by the Joint Commission. Catholic Home Care services more than 2,100 patients in their residences through the acute care agency and over 200 long term patients each day. The Home Care team consists of more than 550 dedicated professionals, para-professionals, and support staff to ensure that patients continue to have their home care needs met twenty-four hours a day, seven days a week.

Catholic Home Care has been ranked within the Home Care Elite list of the top 25% of home care agencies in the country. The HomeCare Elite™ is the compilation of the most successful home care providers in the United States. CHC provides skilled professional and paraprofessional home health care services throughout Nassau, Suffolk, and Queens Counties. Specialty services include skilled nursing, physical therapy, occupational therapy, speech language pathology, nutritional counseling, medical social work, and home health aides. We have program specialties with highly trained staff to include pastoral care, behavioral health, infusion therapy, wound management, palliative care, and telehealth.

CHC disease management programs are designed to target certain populations that require specific individualized care to treat their needs. The purpose of these programs is to maintain clients with complex medical needs in their homes while providing comprehensive care. These programs coordinate care for patients under the direction of their physician and provide ongoing assessment, education, and treatment. These disease specific programs include cardiac disease, diabetes, cerebral vascular disease, total joint management, falls prevention, and COPD.
