We are a Nonprofit 501(c)(3) Federally Qualified Health Center with Patient-Centered Medical Home status. We provide medical, dental and behavioral health services and contract with Lyon County to offer health department services. We accomplish our mission with a sliding-fee scale, extended hours, prescription assistance, interpreter services and free transportation.
